I’m in similar situation too. I’m on PC and my first preorder was on Steam with Phantom Edition bought on 18 august. Since I wasn't interested in the contents of the Phantom Edition but only in the Tombstone Pack (active on my profile thanks to the preorder), I switched to the standard version of EA App.
After playing my new purchase with the SAME account that i used on Steam…my Tombstone was disappeared.
I provided EA with the receipt for my pre-order purchase on Steam, but I haven't received a useful response other than the standard automated reply telling me to restart the game.
