Premium like all DLC is platform Specific and not transferrable or accessible from other platforms. If you want Premium on Xbox you'll have to purchase Premium for Xbox - I play on PS3 and Xbox and am Premium on both platforms so I understand your frustration.
Regarding the online pass if you purchased the game used from Game Stop look at your receipt, the online pass code should be on the receipt because Game Stop now gives out free online pass codes for used games that require them.
If you purchased the game used from somewhere other than Game Stop then you will have to purchase a new online pass for $9.99 USD on the Xbox Live Marketplace.
