Having a similar issue. I preordered the physical copy from Amazon and have been trying to install the game for 3 days with no luck. The Campaign mode is at 100% and I have been able to play through the first mission, but the "Full Game" has never progressed past 41%. I've tried deleting and reinstalling several times with no luck- it keeps freezing at 41%. I've also contacted EA support and was told they would escalate the issue and I'd have a response in TWO TO THREE WEEKS. A little annoyed, hopefully we hear something soon otherwise I'll be looking for a refund so that I can buy something that I can actually use my new PS4 for.
Thanks to those that recommended cleaning the disk. I've jumped from 41% - 50% in the last 3 minutes after taking it out and giving it a quick wipe with the inside of my sweatshirt. Odd, though, since there didn't appear to be anything wrong or any smudges.

Thanks! That was the problem, the disc had a lot of smudges on it, and I just had to clean it.
