My Battlefield 3 Single Player Disc for the Xbox 360 is not loading

My Battlefield 3 single player disc is not working.  Other discs are loading so I am pretty sure it's the disc.  I have printed out the disc replacement form but I need an incident number to be able to do this, according to the form.

    You can put in your case or other support numbers as an example. If you never contacted our support forehand, I would say just spare this line and send it without a reference number.
