A squad wipe happens when all soldiers in the same squad are killed at the same time and none of them are spawned in again at the same time.  Aka, none of the squad members have a soldier is 'live-state' inside the game round.  This will also result in that that squad will drop down on one level of their accumulated soldier perks.  You as their enemy will get the extra points if you are the one killing their last alive soldier and trigger the 'squad wiped' situation and you will get a message to the same on your screen.
