5 WR set in dynasty is putting only ten players on offense, then crashing game.

I'm 4 years into my online dynasty with my friends. New season just started and now whenever I run a 5WR set the offense will only have ten players on the field. Then the game will crash. Or it will send out 5 WR but two of them are actually a linebacker and a safety. And neither of them are in the formation subs to potentially explain why.

In the photo examples below you can see the play I pick and what it looks like once the players are on the field. There's also a strange visual glitch that is combining the slant route into the slots route. About 50% of the time the game will crash whenever this happens. 

Extra details, this is happening on Xbox series X, it's an online dynasty, the season just changed and it started happening first game of new season. I have enough WRs on my roster for this to not be an issue of not enough receivers. The crash freezes the game, then it closes to the Xbox dashboard with no message pertaining to crash.
