Missing Shells in Playbook and formation misalignment

Played a CUT game with 4-3 press Quarters playbook. The 3-3-5 3 High formation is missing the Shells for the entire formation and against certain formations, the slot corner randomly aligns as a MLB...

EA_Shepard​ Auto flip is off the play, reset doesn't work but pressing qb contain moves the slot cb to slot position. The play that is misaligned in the 4-3 press Quarters playbook is the 3-3-5 3 High Edge Blitz 2

Selecting edge Blitz 2 from pre-play or audibling to that specific play causes the slot cb to  move down in between the MLB & DT. Other plays in this formation in this specific playbook are aligning correctly,
