[Old Report] PC - Marker Fragment not spawning

Hi there. Running through my new game plus on Dead Space and as it turns out, chapter fours marker fragment hasn't spawned in.  I've reloaded, turned off, turned on, verified my cache, it's insta...

Is this still a thing for you? I thought it was fixed a patch or two ago?
