Premeditated Malpractice bug - Missing interactions in Diagnostics Lab

At the end of Premeditated Malpractice, I am missing the audio log and other reward gear. The necromorph does not spawn in either. I first tried completing this in Chapter 7/8 and ran into this issue. I then came back in Chapter 10 with the same result. Reloading save, restarting game etc had no effect.
