The absolute most commons cause of this issue is cross program interference. Clean boot sometimes finds this, and others not so much. Known programs that interfere with DAI...AVG/Avast(Some people have reported needing both exceptions/exclusions for DAI, DAI Saves folder and Origin app, AND disabling AVG/Avast to get it to work), Razor mouse/keyboard/headphones install/customization program, TeamViewer, Discord, and Steel series mouse/keyboard install/customization program. I have heard of a few more, usually things that add chat functionality or in some other way link into the game such as Fraps off and on..... Also making sure BOTH Origin and DAI are set to run as administrator and both are using the same Windows compatibility mode....ie IF Origin is set to Win8.1 and DAI is going to use Win10, it has sometimes caused an issue.
You MAY also have a corrupted save file OR settings file, so if you have a Save folder for DAI in your Bioware documents location, you may want to rename it and see if it effects anything.
C:\Users\User Name\Documents\BioWare\Dragon Age Inquisition\Save <---typical location.
Other more drastic thing to try is to create a new user profile with admin rights and load the game and Origin app on it ONLY, and see if it runs. If it runs, then you have a program or settings issue, if it doesn't then you have a quirk in your hardware config that doesn't like DAI and you have a much harder issue to track down if you want to play the game.

I also believe the config file doesn't get deleted when uninstalling the game, so make sure to delete ProfileOptions_profile in the game's save folder. This will force the game to create a new one on launch. This can fix launch issues caused by settings that don't work on certain setups (e.g. if Mantle is set as the API when you have an Nvidia GPU).
