There's a know issue where using Lunge & Slash's alternative upgrade (from the skill tree) make the game crash whenever the skill is used. Might that be the cause of your issue?
I've never really unlocked Lunge and slash on any Warrior, accept for Blackwall this run but I haven't upgraded it.
