Well, seeing as how much small glitches in the game so far, and that no one still made a thread to collect them all, here I go.
I will put here every issue and glitch I have found in the game, I am sure some of them could be reported already, but I do this for the convenience of the other players who could review the problems and add their own, and to make the forum less crowded with multiple topics of the same nature. As well as for the moderators, it would be a bit more convenient for them, should all stuff be gathered in one place.
Naturally, I am playing on PC, those all my posts will be regarded to that platform.
As I told, everyone welcome to add whatever problem or bug they found, BUT if you want to discuss it or do whatever else, beside posting it. Please, make your own thread, no need to pollute this one with arguing, spamming and flaming!
1. The game freezes "occasionally", mostly that happens when you enter new location. That happens allot.
2. NPC, especially your companions and "utility" NPC, at Skyhold, as well as in some other locations occasionally disappear, at all. You have to reload the game, for them to appear again.
3. Sometimes "books" that you find glitches and dos not open a screen with it's content.
4. Sometime, while you play the sounds of battle disappear, you still hear music tho. That usually happens when you already played for a couple of hours.
5. Equipment issue. For example you have a sword with 50 DPS equipped, and you point out a sword with 55 DPS, but statistic calculation dos not reflect that at all or shows that 50 DPS sword is more powerful than 55 DPS one, and that only regarding the bar reflecting weapon damage and armor points (which is most left and lower one). And secondary statistic is equal.
That's it for now. Should I find more, I will update the topic. Moderators welcome to update the status of the issues, that would be appreciated.
2. I'm not talking about repeatable quests..... Main story line quests such as Val roy, enter the city see mother get smacked templars leave, rince repeat.....
3. SAve the game, shut down game properly return 3-10 hours later start game NO SAVES AVAILABLE
SO..... if your NOT going to accept noted glitches why ask for them?
To AriekB: I'm sure PoArVl meant no offence and just tried to guess what was happening with your game, as it does sound odd (I play on PC myself and do not have the behaviour you described); and you have no idea what trivial mistakes people sometimes make, so no need to get personal, we are all here trying to make things better 😉
As for the DxDiag - just something to consider: it is undeniably always HUGE. It would be more readable for the thread if people who wish to share their DxDiag could upload the file to some fileshare and post the link here.
First, there was no need to literally post your DxDiag report, you could just add a link... Second, no one asked me or however posted here for their bug reports, that is our initiative to help fix the game, so if you feel unappreciated, well, Maker help you . And lastly, I was merely tried to be helpful, if that made your pants get in a knot, well, sorry.
Anyway.
67. Mage "Sacrifice" spell, teleports you several meters back instead of blasting enemies. That happens occasionally at "Whistling Wastelands".
68. Mage spell "Rune Blade" of Knight-Enchanter specialization, sometimes bugs. And instead of using the attack movement, it is just makes your character to move forward for a bit, without performing an attack.

69; SOme of the Astranums mini games are uncompletable, there is either miss placed nodes or extra nodes that arn't on the example( between 2 to 3 extra nodes), rendering the cimpletion just based on pure luck 10% of the time and impossible to do 90% of the time.

71. In the Prologue, if you kill the second pair of demons (counting the demons attacking you when you first get your weapon as the first pair)before the "Ability" tutorial pops up (an archer can lure them up the hill, I'm guessing a mage could do the same) the next thing happens when you move further:
- Cassandra says "There, watch out! If we flank them we may gain the advantage" - even though there are no enemies around
- your character unsheathes their weapon and never sheathes it again until you exit to the main menu
-- you can make them sheathe the weapon ckicking "Disengage", but after the next fight the weapon with remain forever unsheathed again
-- you can save during this time, meaning the game does not count you as in combat
PS: To gueric1: not sure if it helps, but I've completed all Astralariums I found in the game, and there are sometimes "extra" stars that need NOT to be connected - just disregard them and draw the correct figure; also (what I did not notice at first) sometimes not all stars fit the screen and you have to drag the camera around to see more of them.

72.War Table: country selection is sometimes not responsive right away. When you summon the War Council and country selection opens - sometimes you cannot click the desired country (just nothing happens), so you have to hover your mouse between Orlais and Ferelden (sometimes several times) until the countries become highlighted.
This one is easier to reproduce when you already have your mouse cursor over the country you wish to select when War Table loads:
a) click Summon War Council door
b) hover mouse cursor over the country you wish to select (for example, Ferelden) while the screen is still black
c) when country selection screen loads - click the country (Ferelden in the example)
I didn't see these mentioned yet. I am playing the PC version with an Nvidia card.
74) Unable to fast travel directly to Crestwood from the Skyhold Undercroft. The name won't show up on or even near the icon, and random clicking does nothing. I've tried during multiple sessions, and it never works.
75) In multiplayer, the game sometimes loads without any of my abilities on the hotbar, and there is no way to add them. Only solution was to leave that game and join a new one. Note that I have only played a handful of MP matches, and only seen this once.
76) Not a bug, so much as a minor inconvenience. I want to see the skulls of the dragons I've slain on my walls somewhere, with a plaque saying what they were. If they each look slightly different, so much the better.
In regards to #47, I think it's working as intended. In my game at least, the ability icon changes when it's active. If I click on it before the duration runs out, the ghost will be unsummoned.
Also, in regards to #72, that stopped happening to me when I started always using the mouse instead of "F" to enter the war room.
77) Weapons should show their speed. With speed we can calculate approximately how frequently a weapon ability will proc. Very helpful in deciding whether the ability is worth more than a weapon with better stats. Also, it would allow us to figure a weapon's DPS against enemies that aren't damaged by any rune on the weapon, since the number shown for DPS assumes the rune is always useful.
