I had this problem and have had it with other games in the past. If you have teamviewer running turn it off. For some strange reason it interferes with some games full screen mode.

This fixed it for me. no idea why, but it worked immediately after I turned off teamviewer.

I don't even have teamviewer, and I'm pretty sure most of the other people in this thread don't either.

in my case I dont have teamviewer and this game is the only game with the problem on my rig.

an update.
My issue was caused by origin itself LOL.
I disabled "in game origin" and now it goes full screen fine first time.

This is because of the "QuickConnect" button that TeamViewer has on by default. If you go to the options menu, advanced settings, click the "QuickConnect" button, then uncheck "Show QuickConnect button", that problem will be no more.
As for me, I also had the problem with Inquisition not wanting to stay fullscreen. In my particular case, it turns out my SSD software, Samsung Magician, was interfering somehow. As soon as I closed it, the problem went away. I can easily reproduce it by starting up Samsung Magician again.
So, those of you who are having this issue: standard disclaimers apply. Close background programs, even those that would seem "important" to you, or that you wouldn't think are related.
