Kudos to middlefngrsalute for identifying what's going on but let's see if we as a community can come up with a temporary measure so we can all enjoy the game to a cetain extent.
I would like for folks with the higher end systems to try this out. I've been tinkering with a variation of the following on my system , ever since I found it while perusing the search engines.
So, step by step to create a user.cgf file
1. Open Notepad
2. Copy Paste this:
WorldRender.MotionBlurEnable 0
WorldRender.MotionBlurQuality 0
WorldRender.MotionBlurMaxSampleCount 0
PerfOverlay.DrawFps 1
GameTime.MaxVariableFPS 60
3. save file as user.cfg and put it into the DA: Inquisition main install directory (the one with the executable)
*Note* The configuration file below is what I use because my system is on the low end of the scale.

RenderDevice.TripleBufferingEnable 0
PostProcess.DynamicAOEnable 0

Oh, and see if disabling the following helps at all.
For Nvidia owners: I found this while reading reddit threads. This is for folks using 344.75 WHQL drivers.
Hoping this fixes my DirectX "Get Device Removed Reason" DXGI_ERROR_DEVICE_HUNG error
EDIT: Improved but not fixed. Was able to play half an hour before crashing instead of ten minutes. Baby steps.
EDIT 2:
I seem to have mostly solved this. Implemented several fixes others have mentioned that made improvements and now, finally, seems to be working. My card is a GTX 650 Ti Boost. So, overall list of changes I made:
Install the new driver that was released today. (This took me from crashing in every cutscene to only crashing in longer cutscenes)
In Nvidia control panel turned ambient occlusion on, to "performance". (This was apparently a fix for this error in Battlefield 3. Not sure if this helped anything or not)
In Nvidia control panel under "manage 3D Settings" set application to "prefer maximum performance". This fix courtesy of AnInfiniteAmount in this thread. Not sure if it helped but it didn't hurt :D
In game graphics options drop tessellation one level and shadows one level. (Individually these both helped. With both I'm now able to play without nearly as many crashes)
Reinstall DirectX from the DAI folder. _Installer\directx\ (this allowed me to ues the war room without crashing)
Things others have had success with that didn't work for me:
Run game in Windowed Fullscreen
Lower the resolution (the game actually scales the UI well; lowering resolution will not give you ridiculously large UI elements)
Turn Ambient Occlusion off in game options
Turn off FXAA
Disable Origin overlay
Disable Origin in game
Possible sound solution from user ipeeblood:
I was having the same issue. After some screwing around, this is what I found:
I disabled all audio devices that i wasn't using (nvidia, realtek, hdmi, etc) leaving only the one device my speakers are connected too, active (a Sound Blaster Z in this case, disabled the spdif just to be safe). For me, this solved the problem. all the skipping, clipping audio disappeared and is normal unbroken sound. Tried it with different sound quality settings and different speaker/headphone configurations and the problem has not come back in two house of play. Hopefully this will work for other people too!
