Problems with codex entries

I have 2 problems regarding codex entries (that I know of atleast).

The first is regarding companion codex images. I completed Dorians quest and in the character selection menu I can see a different image for him. However in the page of codex entries the old image is still used. This is my first run through the game and I haven't been able to verify this with other characters (Though I suspect I may have also not gotten Sera's second image)

The second issue is regarding locked codex entries. After talking to Leliana I got the 'Perils of Bard Life' codex, or so it tells me, the codex entry itself is still locked(entry 106). I have seen that codex during the loading screen though so I know that I did receive it. And the 'Tales' tab for codexes always has the icon to signal an unread codex, despite me reading every codex that I can. So something must be stopping me from getting to it.

I've tried searching for these issues online but I can't find anything, and honestly I have no idea what to troubleshoot. I considered using console commands to force fix these errors but there don't seem to be any commands for this. 

I know these are really minor issues and they don't really detract from my experience of the game, but I was still hoping to make squash this little bug if possible.

    @U1timatum13 Thanks for the post. For the first question it's been a while but I don't think the images change. I'm downloading a copy of the game and I'll load up an endgame save to double-check, but I think this is working as intended. I'll update the thread when the download finishes. 

    Edit: Checking on various saves from various points of progress the cards don't seem to change within the codex menu. 

    As  for the codex entry I'm not sure how to force it to unlock. Sometimes transition areas a few times helps to unstick things.

    Oh that's good to know then. Guess I just have that one bug. It happened around like 50 hours of gameplay ago on this save, so I won't be able to go back to when I was meant to get the codex. Ahh well it's a minor thing and I'm still loving the hell out of this game.
