Random companion damage, out of combat
I have noticed this evening that my AI controlled companions are taking odd damage spikes while exploring out of combat. It's most noticable with low HP characters, which will take 2-3 bars of damage...

Yeah, I see this quite regularly as well (PC - Normal Difficulty). Once I did notice one of my companions randomly shooting up into the sky and falling in a split second taking a ton of damage. But I've seen this happen in virtually every region, the Hinterlands being the worst (for me). Sometimes it's just a little damage... sometimes it's a lot. Once in the Still Ruins I had Cassandra at full guard (and high level) instantly die near the entrance.
