Also having almost this same issue. For me, the book "Way of the Reaver" was lying at Iron Bull's feet, but reading it did not update my quest. I have no idea what to do now.

This did work for me.

If you have the book in your codex and the three primers, go ahead and craft the item you need at the quartermaster. Once it's crafted it will register the book in your inventory and you'll complete the quest. Happy hunting.
I had the same problem with the book close to "The" Iron Bull.
Even tried loading a save to try again, but same results. I'm hoping it'll update properly when the rest of the materials are acquired.
