Did your Warden Romance Morrigan and, do the ritual (have a baby) with her in DA:O? You have to do both then, go through the mirror as well to get the letter you want.

Sure did all of it I even set up the keep that way as well ...
If you have any mods or custom content, recolors of clothing etc... take those out just to be sure that isn't the problem.
If you use Cheat Engine, try without it.
See if it's the same in a new game.
If it is, then right click on the game in Origin and choose Repair. Let that run and test it again be reverting to an earlier save in the new game.
