Varric Keeps Killing Himself?

I'm not 100% sure what's happening but it seems every now and then Varric will just die randomly in combat. At first I thought "oh, he's just getting one shotted by an enemy," but then it happened against low level enemies.

It almost looked like he was attempting to use stealth each time it happens.

    If you check your gear, is he using items with the "berserk" trait? This adds damage to your attacks, but at the cost of increasing the damage you take by 10x the damage you do. So if he is doing 20% more damage, he is taking 200% more damage. This may be why even low level enemies are doing lots of damage.

    Be also SURE, to correct his TACTICS.

    Like how he must fight or doing things while in combat.

    Varric is still great nevertheless, if you use correct skills and tactics.
