War Table Bug - Timed Missions

I am playing on the Xbox 360 and I am about 75 hours into the game.  The Here Lies the Abyss Quest is listed as my active quest.  I have already talked to Hawke's ally in Crestwood, killed the demons and recovered the memories.  II have 102 power and the quest descrition in my journal shows I have 8/8 power required (but the bullet point on this requirement is not marked as met)  It says I am to return to Skyhold to scout the western approach next, but when I go into the war room there is no option to select here lies the abyss.  It is not on the war map at all.  This quest is part of the main storyline.  I can't progress without completing it.  Are there any fixes for this?

    To the west of Orlais, there should be a region marker for the Western Approach, with the fade indicator above it. If I remember correctly you don't actually enter straight into Here Lies the Abyss, you'll need to find Adamant Fortress first. 

    Here's an image of the war table location. Ignore the extra clutter, I only had a later-game save to use.

    This icon and the fade indicator are not present on my war map.  I have tried going back to an older save, because I remember seeing this on the war map at one point, but it is no longer present on any of my older saves either.  Do you have any other suggestions?
