I had this problem when using x360ce with ps3 controller.

It stopped happening after I used a different method(SCP Wrapper with original xbox360 controller driver).

So if you are using x360ce(it does not work anyway), stop using it.

No, I have my Xbox 360 controller directly connected to USB, no other software. A long time ago I briefly had Pinnacle Profiler installed, but I no longer use it.

  I'm not sure, tbh. I've been playing on my personal PC with a 360 pad and I haven't encountered anything like what you've described.
  
  Have you used any other pad/emulation software in the past that might be interfering? If you restart your machine with a Clean Boot do you see the same behaviour?
