XBOX ONE - Settings reset after closing game and several achievements resetting

Every time I start up DAI, all of my settings (such as hiding the helmet) reset back to default, and several achievements are resetting their progress.

The achievement resetting is not limited to a fresh boot up of the game. I had my achievements snapped, and during a dragon fight, I saw my Invincible progress reset to 0. On the Xbox leaderboard as of right now it states "Enemies Slain in Single-Player Mode" 13, yet I have played this game for around 80 hours.

Steps I have taken: Cleared persistent data from my Xbox, uninstalled the game and all DLC, and did a full reboot of the system. I reinstalled the game, repatched the game, and redownloaded all DLC and am still having the same issue.

The issue is following every character I create, so it seems to be linked to my Xbox profile. I tried a separate profile on my Xbox and had zero problems. All settings were saved and achievement progress is not resetting.

It is frustrating have to turn on subtitles, and hide my helmet every time I play the game, but what really gets me is the achievements. I have tried to earn these achievements several times over and they just will not unlock. I have prided myself in unlocking every achievement in every previous Dragon Age and Mass Effect titles, and this honestly ruining a bit of the experience not being able to "complete" this game.

Glitched achievements:

Dragons' Bane - killed 4 dragons and have 20% completion of the achievement. I killed a 5th dragon and went down to 10% completion

Pathfinder - Unlocked all camps, and sitting at 10% completion. I reloaded an old save, and re-setup a camp and went up to 20%. Later it went back down to 10% randomly.

Demonslayer - I randomly lose percentage, or it will reset to 0% randomly.

Invincible - Same as Demonslayer

The only step I haven't taken is deleting every save file on my console. But with the lack of a way to export my character to external storage, and not knowing if it will even work, I have yet to try this step. I feel like some type of settings are corrupted on my profile, but I have yet to be able to figure out how to fix it.

One last thing to note, occasionally all of my save files disappear and I have one file listed as corrupted. It loads just fine, then all of my old files come back.

Any assistance on troubleshooting tips or solutions would be greatly appreciated.
