Community Manager@LunaRayne3405 Hey, so this has been a known issue for a bit. I asked about this about a week or so ago. The team confirmed it was updated a while back. The issue now would be the save files being corrupted as most don't get updated.
So, when it comes to achievements, we can report it but it will be a situation where we find a work around to get it to pop on your account.

Community Manager@LunaRayne3405 The team confirmed the issue was resolved. If you still encounter this issue, it could be a network issue syncing your account or a corrupted save file.
But i have completed the achievment in 3 completely different playthroughs now, and have unlinked and relinked my xbox account and my game account .... i have cleared the cache and everything short of uninstalling and reinstalling the game but inhave 2 other playthroughs that are halfway through and I dont want to lose them
Community Manager@LunaRayne3405 I have asked the team if there are any other options, but it was confirmed resolved, which means Xbox would also have had to confirm it was resolved. I am waiting to see what they say. Publishers cannot grant achievements because those are based on your Xbox profile and the save data for each game, which is what makes these harder to resolve.
We did have one player state how they resolved it on our Dragon Age The Veilguard Technical Issues & Bug Thread.
