I've run into this a few times both prior to and after the first patch - when I go to meditate, it'll play the animation, but once the screen fades to white it stays stuck that way. Closing the game and reopening it and trying again seems to fix the issue, but it's becoming frustrating having to reboot often.
Put all your graphic settings to low before talking to Solas. It worked for me.
My PC is on the lower end of the recommended specs, so I've been playing with low graphics regardless - unfortunately, I don't think that's the issue :(

Also have this issue (After the scene with Morrigan) :(
Thanks 95j52gmf118h, I put everything in Low and was able to start the cutscene ... but crashed after the first interaction (I have the sound, image freeze, and then the game crash).
