Currently experiencing a bug with getting the third “key” in this quest. A ladder is supposed to get knocked down upon companion explosion but the metal never breaks. Tried fast traveling out of the area and came back in, still not working. Issue has been reported on Reddit with others experiencing the same problem.
So I left the area, saved, closed and restarted the game completely and traveled back and it worked! So hopefully that works for you too
Seconding this. I am having the same issue on ps5. Can not progress in wardens vault or wardens grey questlines.

Hello, thanks for the tip but didn't work for me :( I think I've tried everything apart from uninstalling the game.
having same issue , explosion doesn't take metal wall down so no ladder release, even started a new game and when I got there same issue.
And another new game ,Mage this time, and once again the same issue. Last play through I came back to try to finish this and no amount of reloading /saving coming back a week later( several times) and its still broke blocking that whole section of the map from play.
Still having this issue 5 months later
cannot get the ladder to drop, nothing works. EA, cmon.
