Thank you, so below is what i pulled from your DxDiag and steps to take for this one. Keep in mind that even with AMD, the game still uses Nvidia drivers for programs like PhysX, you will see some of the steps for that sometimes as well. Most errors revolved around your GPU drivers so i would do a fresh install of them.
