It does seem like a disk problem. Sometimes it does it randomly while playing it. I can be walking around not fighting or even talking to characters and it just makes a loud crunch noise then sometimes it will say that the app has crashed and other times it says the disk need to be ejected. Then when i go to eject it the ps5 takes it as of there is no disk in and i have to restart the full ps5 and then it lets me eject it. I am going to keep trying it to see if it gets worse or if i can atleast finish the game. I mostly wanted to see if anyone else was having the same kind of issue. And also tried star wars outlaws disk and steller blade disk and they both work fine. So as you said seems like a disk problem.
