It's not a bug; you just need to collect all of the candlehops and the path opens back up.
The Candlehops quest leads you to a different chest (past the barrier and up some stairs). The chest OP is talking about is down the stairs in the Shadow Dragons base, to the sewers (I think it’s Anvallenim) where you originally do the Shadows of Minrathous quest. These stairs are blocked by rubble if you choose to save Treviso.
The other entrance to the sewers, the gate/door with the snakes, is closed after the quest.
The chest is not accessible at this point, unless it’s patched. No other counted chest in Thedas is “missable”, this is the only one in the game. Players are stuck at 99% map completion because of this lock-out.
