Pinnacle of its kind - Primary demon fight does not trigger

It happens when you unsheathe your weapons, there's supposed to be a "boom" but when the dragon didn't spawn for me I didn't get that either.  I reloaded the save and that fixed it.

I'm on PC btw so I don't think it's platform specific.
