Tattered Bridge Puzzle Bug

When doing the Tattered Bridge puzzle to get the chest, it bugs out.  It happened to me when I didn't complete the whole thing, left to do a mission or something else, came back, and it wouldn't continue the puzzle.  See the video for where it is broken.
