Ultimate Skill keeps resetting

Hi all!

Been having a blast with Veilguard. One issue has kept popping up for me and I'm not seeing much discussion of it so I thought I'd post here just in case.

I keep discovering that my Ultimate Skill has been reset to the default one. I'm playing a Champion Warrior, so I'd like to be using Warden's Fire, and every few combats I find that I somehow have Flashing Fists equipped again. I'm generally only noticing this when I go to actually use my ultimate and then notice it's the wrong one, so I'm not sure precisely how often it's happening, but it's happened two or three times at least in the past couple of hours of gameplay.

I'm on PC and have the latest update.

    Confirming that it's STILL happening after Patch 3 and still seems to be related to skill tree interactions.

    Confirming it's still happening.

    And while we're on the topic of **bleep** Patch 3 was supposed to address but didn't, could you please let somebody know that they only fixed the WASD-to-arrows keybinding bug and not the others? For example, it's still impossible to remap Map to most things, including Tab.
