Varric's shaving mirror inaccessible
Myself, and a number of other players have missed the chance to set gender/scar options - either being too eager to get back to the plot line, or because of believing the mirror can be accessed later...

You can change how your Rook looks by using the Mirror of Transformation in the room where Varric is recovering, the room where Rook woke up after stopping Solas in the beginning of the game when you arrive at the Lighthouse for the first time. The Mirror of Transformation is in the corner of the room. Varric's shaving mirror is just a decorative item in the meditation room and is not used for changing how your character looks.
The appearance of Rook and the trans identity flag used in conversations are two separate things.
