I am having this issue too.
I moved from Origin to EA App a few weeks ago (mid-May?) and was surprised to see all my games already installed, as they were with Origin (they are stored on external HD, but my game files stored on my Mac). The game was launching faster (like from 40-45 mins down to about 6, flat) so I've been suspiciously waiting for something to go wrong - as I've heard literally only negative things about EA App.
Unsurprisingly, it's my turn! So, I've bought an old kit, Bust the Dust as a little treat. Denied. EA App already has full disc access enabled, but it repeatedly says it is not. I have tried opening, closing, I've shut down and restarted the laptop - nada.
I am running macOS Catalina 10.15.7. Bust the Dust requires 7.1mb, I have GBs available.

This is what mos recently worked for me, I had the exact same problem! Open Disk Permission, click the Plus, and add the Sims 4 game/app for full permission which I selected from my external hard drive. Then in the same window, I used the search bar for anything containing EA on my Mac. These two popped up, I selected both, for full disk access: EAHelpLauncher and EABackgroundService. I restarted the EA app and then was immediately able to download the newest Kits I purchased. Hope it helps!
