When I am logged in on the playtesting portal and enter my profile, press "Add family member" It leads me to a playtesting survey I assume is no longer active, referring to November and in a location I did not include in my preferences. The browser used was Chrome on a PC.

Have you tested on a different browser to see if this may be a caching issue?
If not, can you do that now and see what happens?
If you check your communications from the playtesting you should see options to speak with someone from the team for assistance.
