Fraudulent account created by hacker

PLEASE DELETE MY EA ACCOUNT AS THIS WAS FRAUDULENTLY CREATED WITHOUT MY AUTHORIZATION.

That was not the solution - I submitted it and have not received anything and had to make up things for the required boxes. WTH.
