Wild Hearts Trophy “Quintessence of Form” Not Unlocking

I got my Tsukumo’s attack form to max level, and I unlocked the Tsukumo hat, but the Trophy for doing so did not unlock. I reset it several times and redid it, quit and reloaded the game, and even tried resetting the internet connection, but nothing made the achievement appear. Can anyone help me figure out what’s wrong?
