Clubs Team Not Accessible Anymore

Hello,

Several months ago i had a team on clubs, due to some problems on my computer, i was not able to play for months and today i see that i no longer own a club. I also tried creating the same team again and got the team name is currently in use error, also lost all my club progression. Does anyone know a solution for this?
