Game goes on PS home menu after a forward through pass

hey ,its been a while, and its also frustrating,

on online Mode Ultimate Team my game play stops and goes to the PS Home Menu after a forward through pass ,at the beginning i thought its my controller but after changing around three controllers the bug is still there .

Please help
