DirectX Crash (DXGI_ERROR_DEVICE_REMOVED / DXGI_ERROR_INVALID_CALL)

Since today's game update introduced the new Penalty mode, my game crashes consistently when playing it. The game runs normally otherwise, but crashes during/after Penalty mode with the following error:
