Fut champs glitch

I was playing my last fut champs game to go 11-4 the I lost on penalties so I went 10-5. Suddenly it disconnected me from ultimate team. And the champs tab showed 10 wins 6 losses and -1 games remaining. So I can’t claim my rewards. I have tried everything. Played a full division rivals match. Closed Fc multiple times. Restarted my Ps5 couple of times. How do I fix this?
