Unable to submit ban appeal, error "Case not created"

Hello EA team,

My banned EA account email: [edit: email address removed]

My account is permanently banned.

When I try to submit an appeal in Penalty History, it keeps showing "Case not created", I cannot create the appeal ticket.

I never used cheats, hacks, scripts or shared my account. I think this ban is a mistake.

Please help forward my case to the ToS review team.
