Franchise Injuries

Even when injuries and fatigue are turned to 0 or Off, all skill positions during the game ends up getting replaced with backups or players not even in that position. Starting RBs, TEs, WRs, CBs. Can anyone explain why this continues to happen? I can’t even play Franchise mode like this?
