If you still have access to the account on your old phone go into the Settings > Credits and look for your UserID (it's at the bottom). Go to the EA Madden Mobile forums (http://forum.ea.com/eaforum/forums/show/3936.page) and send a PM to EA_Pitts (he is an EA rep and has been able to help others). If you can't find your UserID he may still be able to help you if you remember your exact username.
While the method that @CalgaryGuy76 said will probably work, the best method is to get the original phone that you were playing Madden Mobile on, to turn on, sync it to Facebook (even if you have to make an account specifically for madden mobile), then go to the new device and connect to facebook. It's much easier than the other method.
