We've made the following fixes on Xbox consoles in this update. No other platforms were affected. General Fixed an issue that prevented in-game Achievements from unlocking as intended, which blocked players from unlocking Advanced Training abilities.

Nice. It's retrospectively credited bugged Insanity and Paramour achievements on Xbox (and in the game - this screenshot from Mass effect 2).
