Mid 2021 and I’m having this same exact issue. Very frustrating, and even more frustrating that no fix or even a response/acknowledgment has come from EA.
I'm on pc.
Studio support for the game was ended in 2017 about 6 month after launch.
That was announced by Bioware and also got comprehensive reporting.

For the problem itself, try to repair the game.
(Origin > Game Library > right click MEA > Repair)
(Steam > Game Library > right click at MEA > Properties > Local Files > Verify integrity of game files.)
If that don't helps you might want to try try a different squad, it seems to be triggered by certain squad combinations.
