The same issue here. I never received any other code. The only serial I have is the serial from the game itself, but from what I read here, even if you have the code, you have to enter it both inside Origin and bioware page which you can't.
So we need an solution to this problem. It seems like they can't manually add this dlc to our accounts. This dlc is neither in my origin account nor in my bioware account.
Should I have this "Collectors' Weapon and Armor" dlc among others listed in https://social.bioware.com/user_entitlements.php ?

Having the same issue, spent 2 1/2 hours on the phone with support and they said to try a different CD Key.
I tried pointing out that my Origin and BSN don't necessarily match as with the rest I don't show a Collector's Edition for ME2 whereas I do for ME3 and Dragon Age.
Will probably be on the phone with them again tomorrow.
Talking to with one of their specialists, this error is due to BioWare changing their front end on BSN.
They are working on a fix and will update as soon as they can (seeing as they royally borked the system).
If you are experiencing the same issue it is recommended that you call in to their help people and get them to transfer you to the speciallists as they are tracking all issues.
I used the chat system and the person helping me contacted the specialists. He told me that he linked my origin with a new serial number but for some reason I was not able to see that from my side and my game still had the same serial number. I was told to reinstall the game and the problem is still here.
