@EA_Illium I'm not OP, but I've experienced this a few times (two or three after a battle and one in the Citadel walking around C-Sec or the Embassies [forgot which one]). For one of the battle situations, it was fixed after I left the area (I died in the other one because of it). The Citadel scenario was fixed after leaving the area.
UPDATE: I just encountered this bug again after a battle on grissom system for one of the side missions. When I opened and closed the menu screen, the problem was resolved. I didnt have to leave the area.

EA Staff (Retired)@LightYagami245 Did the remaining enemies seem to have any effect on the bug?
- @EA_Illium The only time the enemies impacted the bug was in the first battle I experienced the bug in (forgot which mission it was in, but it was not for the Grissom mission), where I was killed due to that.
Other than that instance, the issue occurred after all the enemies had been killed.
