Everything I've found wrong in ME:A (feel free to add anything I've missed)

I'm going to add a few things here:
When getting the Asari ship - is it me or does the Pathfinder and Captain look like twins. Really?

When I do the Angara training and I talk to the Angara on the Nexus - it won't let me talk to him when done.
I click (hit enter) and it just stays there.

On Kadara - scan the bodies - I scan one body and that's it - I can't scan no more and never get to

finish the quest it's related to.
