This fixed it for me. Using a 144hz 1080p monitor. Disabled VSYNC and reduced framerate cap from 240 to 60.
And in case it's relevant. In the scenario where I crashed, Grunt had Fortify enabled during the cutscene, and Shepard had Power Armor + Barrier enabled. After loading and disabling VSYNC, those abilities were not active.
