Mass Effect 3: Custom Shepard Face turns into Default Shepard

I have played Mass Effect 1 and 2 with a custom Shepard. Upon playing the third game, during the turian moon mission my custom Shepard turned into a default Shepard. The point of me telling you this ...

When I reloaded an earlier save, my Custom Shepard face was there. Another thing to add is that I changed my characters gear on the Normandy early on, but along finishing that beginning moon mission, all my armor customization was reset.

I unfortunately noticed that the face was different after a fee missions and didn't want to have to replay them. I know it was the moon mission as I checked back on my reloaded saves.
